Description:

Homes for the Homeless (HFH) is the largest single provider of transitional housing in New York City. At our five American Family Inns, families receive the education, job training, and support services necessary to maintain permanent housing and become self-sufficient.

The responsibilities of the Case Manager include, but are not limited to:

• Handling a caseload of approximately 20 families
• Developing service plans based on the assessment of clients’ needs and goals
• Working with clients to maintain an open public assistance case
• Assisting clients with referrals to outside agencies for employment assistance, medical services, GED assistance and follow-up ensuring they utilize all available resources
• Maintaining and updating case files to document progress
• Facilitating the flow of information to housing staff

Additional Qualifications:

• Bachelor’s degree in social sciences
• Strong organizational skills and attention to detail
• Working knowledge of Microsoft Office (Word and Excel)
• Excellent written and verbal skills
• Bilingual (English/Spanish) a plus but not required
